Fundamentals of Population Health

The programme provides participants with an understanding of the concepts and objectives underlying Population Health Management. The course provided the principles and insights for sound design in Population Health Management (and its critical differences from clinical care, disease management, and integrated care), the framework to identify challenges and develop steps required for its successful implementation, and the skillsets for the monitoring and evaluation of complex projects. Specific learning objectives include:

  • Articulate the foundational concepts and paradigms of Population Health

  • Analyse and establish sound objectives for Population Health initiatives

  • Analyse and construct sound dynamics for Population Health systems

  • Apply systems thinking to manage complexity and constraints in Population Health

  • Integrate and synthesise evidence for design, implementation, monitoring, and evaluation of policy and programmes

  • Apply appropriate levers to create the biggest impact on the health of the population

  • Appreciate the principles in designing an appropriate monitoring and evaluation framework for Population Health

  • Architect a sound systems approach for improving the Health of the Population
