Certificate of Participation in Introduction to Population Health Management

Improvements in the health of the population require interventions beyond the clinical interfaces. Population Health Management examines the outcomes desired for the health of the population, identifies the determinants of health and their influences and constraints, and then intentionally implements policies and programmes to address the rate-limiting factors. This executive education programme establishes the concepts and objectives underlying Population Health Management. It will provide the principles and insights for sound design in Population Health Management (and its critical differences from clinical care, disease management, and integrated care), the framework to identify challenges and develop steps required for its successful implementation, and the skillsets for the monitoring and evaluation of complex projects. Examples of Population Health Management initiatives around the world are examined for their paradigms and principles of design, and contextualised for Singapore. Participants will receive an electronic Certificate of Participation upon attending and completing the entire course.
